Are you a compassionate and skilled Registered Nurse (RN) eager to contribute to end-of-life care? At the Center for Acute Hospice Care (CAHC), an acute hospice facility with Hospice of the Piedmont, we're seeking dedicated professionals like you for PRN (as needed), 7a-7p and 7p-7a shift opportunities.
As a *PRN Registered Nurse*, your responsibilities encompass conducting comprehensive nursing assessments to discern patient needs and coordinate supportive care.
Your role involves ongoing re-evaluation and assessment of these needs, as well as the management of all facets of terminal care.
In collaboration with the health team, you play a crucial part in ensuring that the final chapter of your patient's life is characterized by quality, compassion, and dignity.
In this role, operating under the guidance of the Clinical Services Manager, you will be a key player in the planning, organization, and direction of inpatient hospice care.
As an RN (PRN), your focus will be on identifying the needs of patients, families, and caregivers, and delivering nursing and supportive care in adherence to the established plan of care and organizational policies and procedures.
*Requirements:*
* RN license in the Commonwealth of Virginia
* Graduated from an accredited nursing school
* 2+ years of experience in hospice, home health, or ICU CCU
* Preferred: CHPN certification
* Basic knowledge of Medicare/Medicaid reimbursement regulations
* Current CPR Certification
* Strong communication and interpersonal skills
* Ability to handle multiple tasks and work well in a team
* Basic computer skills
*Responsibilities:*
*Patient-Centered Care:*
* Conduct initial assessments of patients and families
* Develop and revise care plans based on evolving needs
* Administer prescribed medications and treatments
* Provide emotional support and assist with daily living activities
* Maintain accurate electronic records of patient care
*Effective Communication:*
* Submit accurate and relevant clinical notes
* Collaborate with physicians on patient needs and condition changes
* Educate patients and families on end-of-life care
* Work seamlessly with the interdisciplinary team for comprehensive patient care
*Additional Responsibilities:*
* Supervise inpatient unit CNAs
* Actively participate in quality improvement initiatives
* Ensure availability of necessary equipment and services
